This notice describes how information about mental health practitioners and other professionals is gathered, used, and disclosed by Radiant Interactive Group, Inc. (“Radiant”) in connection with its website services known as BHRACheckup.com, HealthyLivingCheckup.com, BHCheckup.com, WellbeingCheckup.com, and CatalystTherapyPlatform.com (“Service”)

 

What Information Do We Collect?

 

Registration Information. To utilize the Service, you will be required to provide your name, mailing address, email address, and credit card information, and at the option of Radiant, copies of your driver's license or other identification, professional licenses, certifications, malpractice insurance, and your Standard Counseling Agreement and Notice of Privacy Practices.

 

Practitioner Profile. You will be required to create a profile on the site in which you will post your name, professional designation, office address, email address, phone and fax numbers, license information, and educational background, as well as copies of your Counseling Agreement, Notice of Privacy Practices, and various other data pertaining to your experience and practice.

 

Cookies. Radiant, through the Service, collects information about your use of its web site through the use of cookies. Cookies are small pieces of information passed by the web server to your browser. We use cookies to identify you to the web server during page requests and to keep your session active. You can control the use of cookies through your browser settings.

 

How Does Radiant Use and Disclose Your Information?

 

Registration Information. Radiant does not disclose to anyone your residence address, home phone number, credit card information, or the information contained in your personal identification, or your malpractice insurance cover page, unless required by law to do so.

 

Practitioner Profile. The information which you voluntarily provide in setting up your profile will be viewable by all website users and other affiliated practitioners.

 

Background Checks. Radiant reserves the right to submit a mental health professional's personal and practice information to a professional background-checking organization. The organization to whom we submit this information will, like Radiant, not sell or share it with any other party in any way. If you would like to know the name or other information about any background-checking organizations we use, please contact the Website Administrator.

 

Opt In Email. Upon signing up for membership, you are automatically included in our opt-in email list. This means that you will periodically be sent emails, newsletters, announcements, and advertisements from Radiant. Radiant will not sell or provide your email address to anyone, but may send you correspondence containing offers and information from third parties. All correspondence sent to you by Radiant will contain a "Remove Me" link to enable you to opt out of future mailings, if you so choose.

 

Aggregate Information. Radiant may make available to unrelated third parties aggregate information about you. Aggregate information is statistically relevant information based on the overall population of users of the Service and does not identify the particular individual whose information has been aggregated.

 

Future Management. If Radiant becomes owned or controlled by other individuals or entities, the information we have collected may be transferred to such party or parties. Such individuals or entities will be required to follow the terms of this Notice in the place of Radiant’s current owners.

 

Disclaimer. Although Radiant will use all reasonable efforts to safeguard the confidentiality of your personal information, transmissions made by means of the Internet cannot be made absolutely secure. Radiant will not be liable for disclosures of your personal information due to errors in transmission or the unauthorized acts of third parties.
